By using the Vision Screening Protocol© or any of its components, you accept these Terms of Use. If you do not agree to these Terms, please do not use the Protocol.

1. Introduction

Welcome to the Vision Screening Protocol© ("Protocol"), developed by A Shared Vision to help non-medical professionals identify children from birth to age three who may benefit from evaluation by a pediatric eye doctor.

These Terms of Use explain who may use the Protocol and the responsibilities associated with its use.

2. Definitions

• Protocol means the Vision Screening Protocol© and all associated materials, resources, technologies, training, and future updates developed by A Shared Vision.
• Protocol Components. These include, but are not limited to Parent Questionnaire©, Objective Vision Testing (fixation, tracking, pupillary reflex, and corneal light reflex), Scoring Guidance©, Scoring Rubric©, Results & Next Steps for Caregivers©, training materials (e.g., videos, examples, quizzes, and supporting documentation), and associated webpages, downloadable PDF forms, CognitoForms, translations, and related implementation resources.
• Vision Screening Training. This refers to the official A Shared Vision training program delivered through live webinars, on-demand learning, or other approved formats. Successful completion of the corresponding training is required before using that version of the Protocol. Sign up for the training HERE!
  
• Technology. This refers to the electronic tools that support the Protocol, including downloadable PDF forms, CognitoForms, and other current or future technology platforms made available by A Shared Vision in both English and Spanish.
  

3. Your Agreement with Us

By using the Protocol, you agree to these Terms of Use. If you do not agree, you may not use the Protocol.

4. Who Can Use the Protocol

Only professionals who successfully complete the corresponding A Shared Vision training may use the corresponding version of the Vision Screening Protocol©. Organizations wishing to implement the Protocol may obtain an organizational license subject to additional terms and applicable fees.

5. Licenses

Individual License. Upon successful completion of the corresponding A Shared Vision training, you receive a personal, non-transferable license to use that version of the Vision Screening Protocol©. There is no fee for individual licensure.

Organizational License. Organizations may obtain a license to implement the Vision Screening Protocol© subject to additional terms and applicable fees. Organizational licensure does not replace the requirement that each individual provider successfully complete the corresponding A Shared Vision training before using the Protocol.

6. Use Responsibly

Please use the Protocol legally and responsibly. Follow the corresponding A Shared Vision training, accurately interpret screening information, make appropriate recommendations, and communicate those recommendations clearly to caregivers.

Don't copy, modify, redistribute, create derivative works, or incorporate the Protocol or its components into another product, platform, or system without written permission.

7. Ownership and Rights

A Shared Vision owns all rights to the Vision Screening Protocol© and its associated materials, training, and technology. The Protocol is protected by copyright and other intellectual property laws.

8. As-Is Basis

The Vision Screening Protocol© is provided "as is." A Shared Vision makes no warranties regarding its use.

9. Limitation of Liability

A Shared Vision is not liable for any direct or indirect damages arising from the use of the Protocol.

10. Indemnification

If your use of the Protocol results in legal action, you agree to defend and indemnify A Shared Vision and its directors, officers, employees, volunteers, and representatives.

11. Changes to This Agreement

We may update these Terms of Use from time to time. The current version will always be available on our website. By continuing to use the Protocol, you accept these changes.

12. Availability of the Protocol

We may end support for the Protocol or related technologies. If that happens, we'll provide at least 30 days' notice whenever reasonably possible.

13. Governing Law

This Agreement is governed by the laws of Colorado.
