Welcome to our Vision Screening Protocol© ("Protocol"), designed for non-medical professionals to identify children, birth to age three, who warrant evaluation by a pediatric eye doctor. This document, our Terms of Use, outlines how you can access and use the Protocol and related technologies.

By using the Protocol, you agree to these terms. If you don't agree, please refrain from using the Protocol.
4. Who Can Use the Protocol
The Protocol is free for personal use. For organizations, there are additional terms and possible fees to use the Protocol.
5. Licenses
Individual License. Once you successfully complete the Parent Questionnaire© training (at least 80% score on the final quiz), you'll have a personal, non-commercial license to use the Protocol. There are no fees for individual use.
Organizational License. Organizations can also obtain a license to use the Protocol after agreeing to additional terms and possible fees. For more information, including details on HIPAA compliance, PHI protection, and language translations, contact us at [email protected]. 6. Use Responsibly
Please use the Protocol legally and responsibly. Don’t modify, redistribute, or create derivatives without permission, and make sure not to overload our networks.
7. Ownership and Rights
A Shared Vision owns all rights to the Protocol and related resources, trainings, and technologies. It’s protected by copyright and other intellectual property laws.
8. As-Is Basis
We provide the Protocol "as is." That means we don’t offer warranties of any kind regarding its use.
9. Limitation of Liability
A Shared Vision is not liable for any direct or indirect damages that result from your use of the Protocol.
10. Indemnification
If your use of the Protocol leads to legal action, you agree to defend and indemnify A Shared Vision and our team.
11. Changes to This Agreement
We may update these terms occasionally, and we’ll post the new version on our website. By continuing to use the Protocol, you accept these changes.
12. Availability of the Protocol
We may end support for the Protocol or related technologies, but if that happens, we'll let you know at least 30 days in advance.
13. Governing Law
This Agreement is governed by the laws of Colorado.
